COVID-19 Reporting Forms

Click on the reporting link below that best describes the scenario that applies to you:

• You have received a POSITIVE test
• You have had a CLOSE CONTACT EXPOSURE to someone with COVID-19, and/or you have SYMPTOMS but have not yet tested positive. 

If neither of these scenarios apply to you, you do not need to submit a report at this time.
